You must be enrolled in a high deductible health plan to qualify. To determine if you qualify for an HSA account, fill out an HSA Eligibility & Information Form. To open an account, fill out a deposit account application and bring it and the eligibility form to one of our locations or just stop by and we will complete them for you.
